Coaches BCI FormThis page was last updated on Wednesday, October 24, 2007 at 10:34 Coventry Soccer Association requires that all coaches/Adult Volunteers that work with our youth MUST fill out the attached BCI form (Bureau of Criminal Investigation) and submit it to the Director of Coaches. The Director of coaches will then give all forms to the Coventry Police Department to conduct a Criminal Background check. If the Police Department finds anything that would warrant the coach/volunteer to be rejected by CSA, the coach/volunteer will be informed that they will be unable to coach/volunteer within the CSA League. CSA will not be given the reason as to why the BCI was rejected as unfavorable, just that it was unfavorable.
IMPORTANT NOTICE: Apparently there is some concern about Identity theft, with the Coventry Police asking for SS#, on the BCI form. Click here to read a letter from the Coventry Police Department, which list the requirements' for them to do a BCI.
You may type into the form prior to printing so that all your information is clear, but please make sure that you verify ALL information by RECHECKING before printing.
